Enrollment Information

Returning Students: (including all NEW 6th grade students from Englewood elementary schools) will register at Flood Middle School on Tuesday, August 1, 2006 from 9:00 a.m. to 4:00 p.m.

Families NEW to Englewood: and Flood Middle School are asked to call Flood at (303)761-1226 to obtain registration information. Students should accompany their families to registration and should bring the following documents:

1. Proof of residency (ex. lease, phone or electric bill)
2. Previous school records (report card, CSAP scores)
3. Legal guardianship/custody papers (if not living with natural parent/s)
4. Special Education records if applicable (IEP)

1. Proof of residency: Regardless of resident or non-resident status residency must be provided. This may include: Public Service bill, driver's license, telephone bill, or lease agreement (not handwritten).

2. Previous school records: Include recent grade card, transcript, immunizations and attendance records. If a student is enrolling during the school year he/she must bring "withdraw" grades from the previous school.

3. Custodial papers: If the student resides with less than both natural/adoptive parents, a copy of the court ordered custody must be provided. If the custodial parent does not live in the Englewood Schools then the student could be viewed as a non-resident student.

4. Special Education Student: If the student has been enrolled in any Special Education classes (has an IEP), then a copy of the IEP must be obtained from the previous school, prior to admission.

Entry Application: This must be completely filled out and signed by both the parent and the student. If the student is a non-resident, then a Non-resident contract will also be needed and filled out.

A new student that speaks little or no English will meet with the ELL teacher and be tested for appropriate placement.

It is the parent's responsibility to bring or to have the previous school fax unofficial records and information needed for the new student.
